- Your name, telephone number, and extension
- Specific part of the software that may have caused the problem
- Error message(s) received
- Exact steps to reproduce the problem
- Frequency of the problem
- Operating system release and AVOLENT product version
- Any documentation that may help in resolving the problem (this could include log files, environment variables, configuration files, etc.)
